CHA - Community Health Assessment

The Columbia Gorge Community Health Assessment focuses on the region overall and supplanted independently conducted health assessments by hospitals, counties, health departments, hospitals and county mental health agencies.

CHIP - Community Health Improvement Process

The CHIP identifies the top focus areas for the region. The Community Advisory Council (CAC) led the development and prioritization of the focus areas using broad input from the community.

The Quality Incentive Measures summarize overall performance in the Gorge CCO on Healthcare, Customer Experience and Cost.
