Struggling to access resources and services on your own?

A CHW can inform, connect and guide you through complicated systems so that you get the assistance you need. If you are interested in working with a CHW, fill out the referral form linked above.

Bridges to Health is a program of the Columbia Gorge Health Council that has been serving the Hood River and Wasco counties since 2016.

Community Health Workers (CHW) are members of the communities they serve and work in team environments. They build trusting relationships with community members, local social service agencies, and healthcare providers. The CHW workforce models and promotes health equity for communities.

Bridges to Health kindly accepts donations to our Client Incidentals Fund which goes directly to clients to help with housing fees, food, clothing, services, needs unique to each client to help them through tough times. We are a program of the Columbia Gorge Health Council, a 501(c)3.
